RSVP Cancel Form – customizable template

I use this form for reservations:

<tr><td>#_CAPTCHA</td></tr>

<tr><td>#_SUBMIT</td></tr>

</table>

All is ok while in reservation mode, but after reservation the submit button to cancel the reservation don’t stay in TD and goes out of the table.

Is it possible to have customizable template for cancel form too?
