Shortcode for Templates

Can not find the documentation on short codes for templates. What code do I use to call a template on a page?
I have tried [eme_template_id=4], but that does not work.

You can’t use templates on pages, they are to be used in other shortcodes or options (by providing the id – see doc on shortcodes – or selecting the template from the list).

Thanks, I am still not getting anywhere.

Here is what I have done:
1. Created a template (#4) with the following
[eme_add_member_form]
#_LASTNAME
#_FIRSTNAME
#_EMAIL
#_SUBMIT

2. Then on the page I called:
[eme_add_member_form template_id=4]

3. The preview page is blank except for the page title.
